COVID-19 Testing at Nativity

Nativity School, Los Angeles is offering free COVID-19 testing to our faculty, staff and students on the following dates:

Friday, August 13th

Tuesday, August 24th
Tuesday, August 31st
Tuesday, September 7th
Tuesday, September 14th

Tuesday, September 28th
Tuesday, October 12th
Tuesday, October 26th

As shared with all community members, COVID-19 testing is required weekly for the first full five weeks of schooling and then every other week until further notice. Individuals who opt of testing through Nativity must provide a negative COVID-19 test result the first day of school, August 18th and then on the testing Tuesdays that follow (refer to schedule above.)

Parents who choose to have their students test through Nativity should sign the “Test Consent” form located below. Additionally, a copy of the student’s medical insurance card is also required. If a students does not have a medical insurance card, please also complete the “Uninsured Claims Program” form located below and provide a copy of the parent ID.
